Malignant testicular tumor in a sixteen-month old boy: case report

Abstract

Malignant testicular tumors in children are rare. The aim of this paper is to describe the clinical presentation, diagnostic approach, and treatment of testicular tumors through a case report of a boy with an endodermal sinus tumor.
    Case report: A sixteen-month-old boy was referred for a pediatric evaluation due to painless scrotal mass. Physical examination revealed a hard, painless, palpable mass in the right scrotum, and laboratory tests showed high levels of serum alpha-fetoprotein. Ultrasound and computed tomography of the testicles, pelvis and abdomen demonstrated a testicular tumor with neovascularization. A radical right-sided orchiectomy was performed. Histopathological analysis established the diagnosis of a prepubertal yolk sac tumor (endodermal sinus tumor). The boy was regularly followed-up clinically, with laboratory tests and ultrasound imaging. Four years after surgery, he is in remission, with normal growth and development.
    Conclusion: The suspicion of a malignant tumor in a boy with testicular enlargement, combined with prompt laboratory and imaging evaluations, allows for the best possible therapeutic approach and outcome.
